Traffic Collision Report Form
Traffic Collision reports can be taken utilizing the Santa Barbara Police Department Citizen Traffic Collision Report form. Citizen traffic collision reports may be completed by a citizen when any of the following situations exist:
- The report is a property damage collision made for insurance purposes only. (Includes highway or private property).
- An injury collision with no medical treatment or hospitalization. (Includes highway or private property.)
- Hit and run reports with no anticipated follow-up investigation. Criteria for follow-up investigation include the ability to identify both the suspect vehicle and the suspect vehicle driver.
Reports submitted on Citizen Traffic Collision Reports will not be assigned to the traffic division for any follow-up investigation. These reports are for documentation purposes only.

After downloading and completing the Citizen Traffic Collision Report, you may take or mail the report to the following address for processing:

Please allow 5 to 7 working days for Records staff to process the report.
Mailing a Citizen Traffic Collision Report will not result in a police response and is for documentation purposes only.
To obtain your case number, dial 805-897-2355. When calling you will need to reference the “Date of Occurrence” you also have the option of submitting a self addressed stamped envelope and a courtesy copy of the report with the case number included will be mailed to you.
